Week 9 Journal

 

An advance directive is a legal document that defines a patient’s wishes for medical care. This document is a way for patients to share their wishes with family members and health care providers when their illness or mental capacity prevents them from making decisions. As an advanced practice nurse who has care discussions with patients and their families, you need to not only be familiar with the process of completing an advance directive, but also understand how this document might impact your role in patient care and treatment.

 

Journal Entry Part 1

 

For the first part of your journal entry, reflect on the Five Wishes presented on the Five Wishes website and PDF and complete your Five Wishes. Explain your state’s requirements for advance directives, including whether your Five Wishes can be turned into a formal document. Journal Entry Part 2

 

For the second part of your journal entry, reflect on geriatric patients from your practicum site with disorders related to specialized areas of care, such as oncology, nephrology, urology, gynecology, and neurology. Describe a case of a frail elder patient who must make decisions related to specialized areas of care. Then, explain potential patient outcomes and include whether treatments would be beneficial and how they would impact the patient’s quality of life. Finally, describe the patient’s wishes in terms of treatments and interventions for the disorder (Was there an advanced directive?) and how the patient might want to spend any remaining time. Include how environmental factors, such as family, caregivers, ethnicity, culture, religion, and/or personal values, might impact decision making for treatments and interventions. If you did not have an opportunity to evaluate a patient with this background during the last 9 weeks, you can select a related case study or reflect on previous clinical experiences.

 

 

 

Week 10 Journal

 

Elder abuse is any intentional or negligent behavior that results in physical, sexual, emotional, pharmacological, and/or financial abuse. Unfortunately, abuse is sometimes difficult to identify, and elders are often hesitant or unable to report abuse themselves (Administration on Aging, n.d.). This makes it important for health care providers to regularly monitor for signs of abuse and react according to local and state laws.

 

Journal Entry Part 1

 

For the first part of your journal entry, select and describe one of the following types of elder abuse: physical, sexual, mental/emotional/verbal, pharmacological, or financial. Explain strategies for identifying this abuse. Then, explain how you would proceed based on your local and state laws once you have identified this abuse in a patient. Describe resources in your community that assist victims of this type of abuse and explain how you, as the advanced practice nurse, might apply these resources for abused elders who are identified in the clinical setting.

 

Journal Entry Part 2

 

For the second part of your journal entry, reflect on the different cultures—ethnic, religious, regional, or generational—of the elder patient population at your practicum site. Describe the cultures you have observed and explain the influence of these cultures on the prevalence and detection of elder abuse. Then, explain how culture might impact the way you would implement resources for preventing or addressing abuse.

 

Demonstrate and apply knowledge related to disseminating project findings/data with organizational leadership and change agents.

In collaboration with your approved course mentor, you will choose a specific evidence-based practice proposal topic for the Capstone Project. Consider the clinical environment in which you are currently working or have recently worked. The Capstone Project topic can be a clinical practice problem, an organizational issue, a quality improvement suggestion, a leadership initiative, or an educational need appropriate to your area of interest as well as your practice immersion (practicum) setting. A combination of 100 “supervised clinical hours” in community health and leadership areas will be obtained through the application of the objectives listed in the “Guidelines for Undergraduate Field Experiences” manual and as captured in the ISP.

 

Practicum immersion experiences are required in a community health setting. Community health and leadership practice immersion can occur in the same site and in conjunction with the evidence-based project in the NRS-490V course. Definitions of community-based settings should encourage community integration and involvement; expand accessibility of services and supports; promote personal preference, strengths, dignity; and empower people to participate in the economic mainstream. Examples of the integration of community health, leadership, and an EBP can be found at Healthy People 2020.

Throughout this course, you have been developing a workflow redesign that could be applied in a health care practice setting. However, determining an appropriate workflow redesign is only part of the process. It is much easier to design something on paper than it is to actually make it work effectively within an organization. Nurse informaticists must consider how the workflow redesign can be implemented in a real-world organization. Attention needs to be focused on understanding organizational risks, organizational culture, and the needs of the end user. Implementing a new project may be disruptive. The key is to plan in such a way as to minimize this disruption as much as possible.

As a graduate student and health care administrator, you must be able to research a critical issue or situation and present your findings in a compelling manner. For this Assignment, as well as this module’s Discussion, you will examine the continuum of care a patient may need and research the services available in your area to support the patient.

Note: During the Module 3 Discussion, you will share some of the information for this Assignment with your colleagues.

To develop the content for your presentation, record detailed notes as you complete the following steps.

  • Review the information about the continuum and coordination of health services presented in the Learning Resources, including the HIMSS resource, Continuum of Care, and the tables in Chapter 1 of the course text.
  • Develop a brief description of a hypothetical adult patient who has a serious health problem or injury. Imagine this patient lives in your local area. If you are an international student, you may choose a geographic area in the United States or use your own location.
  • Use the pertinent information in Chapters 7, 8, 9, and/or 10 of the Shi & Singh textbook and conduct additional research of your own to analyze specific services your hypothetical patient might need, beginning with the onset or diagnosis of an illness or injury and concluding with an end-point of your choosing (i.e., recovery, symptom management, end of life). Also research the types of organizations that provide those services.
  • Investigate the health care organizations in your local area. Based on the services your hypothetical patient needs, select four or five actual settings in which he/she would receive care.
  • Next, gather information related to the types of insurance from which each organization accepts payment. Typically, this would include Medicare, Medicaid, and various types of insurance programs. What challenges do you anticipate your patient may have related to payment for services?
  • Review the Institute for Healthcare Improvement (2014) resource and consider what it means for health care to be safe, effective, patient-centered, timely, efficient, and equitable. Analyze how the scenario you have created—with your hypothetical patient receiving services from actual settings in your geographic area—illustrates positive or negative attributes related to two or more of the six Institute of Medicine aims.
  • Next, prepare to create your presentation using PowerPoint. Examine the Presentation Guidelines and Tips and the Module 3 Assignment Template, both found in this module’s Learning Resources.
