Scenario 1: A 7 year old child is tested at school during a group IQ testing and is assessed to have an IQ of 67. She is having a hard time keeping up with her class is reading and arithmetic. She appears to be immature in her social development and needs support with daily living skills such as proper dressing and grooming.

Scenario 2: The lawyer of a young man, Albert S., suggests that Albert see a therapist because he has been so depressed about his impending divorce. Albert says he has been feeling sad, lonely, and fatigued during the last month. You determine that Albert is not suffering from a severe mental disorder; and a physical exam indicates that Albert has no physical problems.

Scenario 3: Gary, a 30 year old man, is known to be quite a “storyteller.” He often brags about his accomplishments, both real and imagined. He passionately enjoys being the center of attention.  Although he expects his circle of friends to be completely loyal, he lacks empathy and understanding in his relationships.  Gary is preoccupied with his personal appearance, and has a fear of growing old.

For each scenario, the following should be answered: (1) what is the DSM-5 diagnosis; (2) what criteria did you use to come up with this diagnosis; (3) what other diagnoses did you consider and why (List 3); (4) list three (3) short-term treatment goals that you would outline in this client’s treatment plan; (5) list three (3) long-term treatment goals that you would outline in this client’s treatment plan; and (6) list three (3) treatment approaches you would use with this client.

Review the Final Project Case Studies found in the Learning Resources for this week and choose one of the four case studies to use for your Final Project. School counselors should choose Case 1 or 2.

Thus far you have learned how to efficiently explore the counseling literature utilizing Boolean search strategies and becoming oriented to current trends in research. You have also gained insight into the ethical and cultural responsibility of applying empirical evidence into your counseling work. It is now time for you to identify the technique or intervention you would like to investigate for you Final Project.

For this week, you will review the Final Project case studies, Case #3 for your Final Project. As you consider case study #3, you will identify one mental health concern and choose a counseling intervention or interventions to investigate for possible use with your client or student.

Also, you will establish a research question related to your case study and selected proposed intervention and then you will conduct a preliminary search of the literature. For your Final Project, you will identify current professional counseling literature related to the selected intervention, analyze the literature, and then apply the findings to the case study.

 

As you learned in Week 1, for the Final Project, you will research one counseling intervention in relation to an identified mental health issue and apply your findings to a case study. This week, there are a number of decisions for you to make to solidify your project. You will complete Part I of the Final Project Worksheet.  The first step is to choose the case study (case study #3) that you plan to utilize in your project. Your case study may include more than one potential client. After you identify the case and choose the client, identify one presenting problem or mental health issue for counseling focus.

Now, put your counselor hat on. You have an identified client and a presenting problem. Where do you go from here? As a counselor, you have an ethical responsibility to provide competent practice. There is an expectation that you will utilize interventions that are supported with empirical research.  So, your next step is to identify an intervention that you would like to use with your client.  For your project, you will investigate current counseling research related to the intervention in order to make an informed decision about evidence based practice. To launch your research, you will formulate a research question to guide your investigation.

Ethics, Subjectivity, and the Art of Diagnosis

Although psychology prides itself on being a science, the art of diagnosis goes beyond a simple checklist of symptoms. Clinicians generally recognize that any diagnostic exercise must include a culturally sensitive interpretation and analysis of symptoms. Considering that only a bachelor’s degree in psychology does not prepare you to do diagnosis, it is important to consider how having this knowledge could support you in other careers. It is also important to consider that even a doctorate in psychology does not license you to treat with prescription medicine. To perform this level of diagnosis and treatment, you would have to earn a medical degree in psychiatry. Sally Parker, OD consultant, and  her team decide to create the Cuyahoga River Valley Organization (CRVO).  The two key questions to be asked in this case study are, ″What should  the design of the new organization be?″ and ″How should Sally go about  implementing the design.″  Be sure to review this week′s resources carefully.
