However, for patients to come to terms with their diagnoses and adhere to treatment plans, they must have an understanding of factors that might have caused, or continue to impact, their disorders. As an advanced practice nurse, it is important that you are able to explain disorders, associated alterations and symptoms, and changes that might occur within your patients’ bodies.
To Prepare

Review this week’s media presentation with Dr. Terry Buttaro. Reflect on the importance of developing an in-depth understanding of pathophysiology.
Select a disorder from the following list:

Adrenal insufficiency (Addison’s disease)
Atherosclerosis
Cholelithiasis (gallstones)
Colon cancer
Cystic fibrosis
Hemophilia
Nephrolithiasis (kidney stones)
Osteoporosis
Parkinson’s disease
Tuberculosis

Select one of the following patient factors: genetics, gender, ethnicity, age, or behavior. Reflect on how that factor might impact your selected disorder, as well as potential associated alterations and symptoms.
Identify the pathophysiology of the associated alterations, including the normal and altered cellular function. Consider both intra- and extra-cellular changes that occur.

BY DAY 3
Post a brief description of a patient scenario involving the disorder and the factor you selected. Explain how the factor might impact your selected disorder, as well as potential associated alterations and symptoms. Finally, explain the pathophysiology of the associated alterations, including changes in cellular function.

Imagine the pain, suffering, and frustration patients feel when they find out they have been misdiagnosed with a disease or disorder. Additionally, consider the financial implications and consequences for time-sensitive disorders such as cancer. Since most disorders have multiple symptoms, and these symptoms are often prevalent in more than one disorder, it can be difficult for health care providers to diagnose patients correctly.

As an advanced practice nurse making decisions related to diagnosis and treatment, you need to have an in-depth understanding of the pathophysiology of disorders, as well as factors that influence them. This week, in preparation for your role in evaluating patients, you examine the impact of patient factors on disease. You also explore the pathophysiology of disorders and resulting alterations in cellular function.

In this first Learning Plan you will review today’s long-term care system and the ever evolving changes. It is a system struggling to respond to a rapidly changing environment with new methods of delivering services and a need to address its lack of coordination and uniformity. You will define Continuum of Care and study the ideal long-term care system, look at informal care giving, and interview three individuals participating in the world of continued care. The interviews will take time to organize so begin this at the start of the Learning Plan to give you time to complete. This exercise should help you understand the issues and concerns of those involved in long term care.

LP1 Assignment: Caregiving Interviews

This assignment will assess the competency 1. Analyze basic measures of health status and health services utilization as a continuum.

This project requires you to interview three different individuals involved in long term care. It is an important activity and time intensive.

A. Begin this assignment with an introductory overview of the different types of caregivers, their roles, and the availability of payment (if any) from third-party sources, such as insurance, for these caregivers’ services.

B. Interview the following three types of people:
• A care recipient.
• A caregiver who is not being paid (family member or friend of the care recipient).
• A professional who provides care to people with long-term illnesses (ranging from physicians to in-home assistants).

Note: Interviewing people who are part of the same dyad is not necessary. If a care recipient/caregiver pair is interviewed, each person should be interviewed alone so that answers are individual.

These interviews can be completed in person, over the phone, email, Instant Messaging, etc. However, it’s best if you can actually speak to the person.

C. Use the following sets of questions for your interviews: Questions for Person Receiving Care, Questions for In-Home Caregiver, Questions for Professional Provider.

D. Conclude the project with a critique answering the following questions, and taking into account all of the segments of the continuum of care studied to-date:
• Is the client receiving the care he/she needs?
• Are the caregivers able to provide the care needed? Why or why not?
• Is another level of care needed? Why or why not?
• What have you learned about the existing system of long-term care?
