Case Study: You are assessing the payer–patient mix for a health care organization. Currently, your payer mix is 40% Medicare, 10% Medicaid, 25% traditional indemnity insurance, 20% managed care, and 5% self-pay patients. Complete the following:

  • Using the most common office visit, CPT code 99214, determine the reimbursement from the Centers for Medicare and Medicaid Services (online fee schedule available for Medicare).
  • Using the same CPT code, 99214, determine the reimbursement for Medicaid (fee schedules should be available from the individual state).
  • Using the same CPT code, 99214, create at least 3 other traditional indemnity insurance reimbursements. If possible, use the actual reimbursement from the insurance carrier. It may be possible to obtain actual reimbursement information from your personal insurance carrier. If the information is not available, assume reimbursement by traditional indemnity insurance is usually 200% reimbursed more than Medicare and Medicaid, and managed care is usually 133% more than Medicare and Medicaid.
  • Compose an accounts receivable benchmark from this information showing columns for current, 30–60, 61–90, 90–120, and greater than 120 days.
  • Assess the information for areas of improved reimbursement of at least 20% or more.
  • Evaluate the options available to change the payer–patient mix with consideration of related legal and ethical issues.
  • Propose a best strategy with justification and rationale based on effective decision-making tenets.

The body of the resultant paper should be 10–12 pages. As early childhood educators, it is our job to ensure that students are learning and developing at an appropriate rate. As we have learned throughout this course, using developmentally appropriate assessment is one of the most efficient ways to be able to ensure that this happens. While it is our responsibility to understand the assessment process and the various strategies, tools, and procedures that are used with young children, the families of the children we work with may not always understand why we are doing what we are doing when it comes to assessment. As the director of an NAEYC-accredited preschool, you have decided to develop a comprehensive strengths-based and family-centered assessment plan where families are included each and every step of the way.

For this assignment you can choose to create your assessment plan in many formats. You could develop a handbook using Word, create a PowerPoint Presentation, or even create an assessment website using a tool such as Weebly. Regardless of which format you choose, your strengths-based and family-centered assessment plan must include the following sections:

The age differences between the dogs is upsetting to the older dogs who are beyond puppy behavior.  In addition, some breeds are less tolerant of other breeds in small spaces.

The objective is for dogs to learn basic commands from their owners, therefore the teacher must offer an environment that is conducive to learning.  There cannot be extensive distractions (other dogs) to keep the dogs from learning the command, therefore a smaller class will be more conducive to learning.  The teacher must model for the clients (owners) how to command the dog, how to reward the dog, and how to follow up to ensure mastery in a positive, engaging environment.  The teacher must guide the clients (owners) the proper way to socialize their dogs within the group.

If shelter dog were brought in to be trained, all the above groups would benefit.
I would suggest the BACS works with the Senior Center to find willing participants to help train.  I would specifically suggest bringing in small dogs who are beyond the puppy stage, so they are more manageable.  My intent would be that one or more of the senior would chose to adopt a dog and have a new companion.  Also, those that have had dogs in the past would be patient and understanding with dogs who did not have owners.  A friendship could blossom.

I would suggest for the child-centered groups that the children work with an adult to help train the dogs.  This would offer the child more responsibility which would lead to increased confidence.  According to the American Academy of Child & Adolescent Psychiatry (2013), A child who learns to care for an animal learn to treat their peers with the same kindness.
