One major point is that we do not prescribe medication, we prescribe psychotherapeutic solutions as we are not medical providers. Also, a clinical justification relies upon DSM criteria as the foundation in understanding the particular symptoms being exhibited. One then matches symptoms to the criteria discussing both in the descriiption. For treatment plans and discharge plans: 1. Goals/objectives need to be measurable ( ie “….will learn 3 grounding strategies to help with…”) 2. Goals are the broad topic and objectives are specific ways to meet the goal. For example, “Goal: Eliza will decrease symptoms related to AUD in order to improve overall functioning. Objective 1. Eliza will identify 3 alternative activities to engage in to manage stress/temptations. 3. Interventions are the specific strategies used in the counseling session to work towards the objective. For example: cognitive restructuring, journaling, social skills training, daily thought record, relapse prevention plan etc. CBT is the modality and is not an intervention in an of itself. The discharge plan is meant to describe what will follow treatment in terms of aftercare. Attention is to be paid to housing, treatment recommendations, medication support, support groups, support system. For instance “Upon completion of treatment as measured by meeting the treatment goal(s) Eliza will discharge to her home with her family. She will begin therapy with therapist to continue to work on strategies to avoid problematic drinking. Eliza will also continue medication services with _______ as an appointment has been set for _______. ” Discharge plans usually include progress that was made, areas that still need to worked on, and the current diagnosis.
