DISCUSSION QUESTIONS

4. Outline a usability test for one module of your current EHR. Include the elements discussed in the chapter in your proposed usability test. Describe why you chose the methods you did.

5. Your organization is planning to purchase new physiologic monitors for the adult ICUs. You are the clinical/informat- ics person assigned to this project. Describe how you would include usability in the purchase process. Outline participants, methods, and tasks to be tested.

CASE STUDY

A tertiary care center in the western United States has an installed base of EHRs supported by Cerner Corporation for inpatient areas and by Epic for outpatient areas. Other technol- ogy includes a suite of about 300 different applications sup- ported by the IT department. The current environment, while including robust capabilities such as computerized pro- vider order entry, is “siloed” with information. Healthcare pro- viders complain that they have difficulty obtaining the “big picture” of the patient across systems and they have to remem- ber information located in disparate systems. They are bur- dened with integrating information themselves. Not only is this time consuming; it is potentially prone to error. Providers have developed numerous workarounds to the different sys- tems in ambulatory and inpatient areas, including “ shadow” files for patients they see frequently. Nurses complain that they have to “jump around” the inpatient system to find information they need for activities such as patient handoffs. The organization responds by developing a vision for the future that centered on the concept of knowledge manage- ment (KM). This concept is defined as the systematic process

of identifying, capturing, and transferring information and knowledge that people can use to create, compete (with other organizations), and improve. A crucial aspect of KM is improving the user experience. As the leaders in the organi- zation begin to address KM and improve the user experience, they are employing the same tactics described in this chapter.

Discussion Questions 1. Assume that you are the leader for UX for your discipline

in your organization. Where would you start to improve the user experience?

2. Pharmacists supporting the ICUs are asking for your help to improve their situation (their user experience) because they are forced to use nonintegrated systems. What methods would you use to examine this issue?

Analyze processes related to cardiovascular and respiratory disorders

Using the below listed case scenario you are to create a 1-2 page paper (excluding the title page and reference page). It is important that you review the APA sample paper and APA requirements for writing your paper.

11-year-old boy complains of wheezing and difficulty “getting enough air.” Notices it more when he is playing baseball and symptoms improve when exercise activity stops. He says that the symptoms are getting worse and the symptoms are even occurring at rest. Mother says the child is allergic to cat dander and his next-door neighbor in their apartment building recently began sheltering cats for the local humane society. Auscultation demonstrates wheezes on forced expiration throughout all lung fields.

Be sure that you are addressing the following within the body of your paper:

  • Analyze processes related to cardiovascular and respiratory disorders
  • Analyze alterations in the cardiovascular and respiratory systems and the resultant disease processes
  • Analyze racial/ethnic variables that may impact physiological functioning
  • Evaluate the impact of patient characteristics on disorders and altered physiology

Conceptual Relationships And Assumptions

In Weeks 3 and 4, you conducted a concept analysis. As Walker and Avant (2019) explain, “concepts allow us to classify our experiences in a meaningful way both to ourselves and others….The ability to express a relationship between two or more concepts is even more useful and efficient” (p. 63). In this Discussion, you create statements about the relationships between two or more concepts, which are critical for developing your theoretical foundation for a program of research.

In this Discussion, you also examine assumptions that may influence your program of research. Assumptions are another type of statement; these statements are considered true, even when they have not been tested. They often go unrecognized because they are deeply embedded in thinking and behavior, so it is important to engage in critical reflection to identify them.

In addition, during this Discussion you consider how nursing’s metaparadigm relates to the theoretical foundation you are developing.

To prepare
  • Continue to review the literature that features concepts related to your phenomenon of interest. As you read each article, consider the relationships between and among concepts. Also identify assumptions that are implicit or explicit in the research.
  • Select one article from your literature review and apply the seven steps of statement analysis that Walker and Avant (2019) present in Chapter 11 of their text. What insights does this give you about the relationships described in the article, as well as for your own work?
  • Think more deeply about the conceptual relationships that may distinguish the theoretical foundation for your future program of research. Also, analyze assumptions that may influence your research (McEwen and Wills, 2019, p. 81).
  • Review the information on nursing’s metaparadigm in McEwen and Wills (2019, pp. 41–45). Consider how the metaparadigm concepts of patient, health, nurse, and environment relate to your theoretical foundation.
