Case Study 5–6 Develop a Motivation Plan Jane Couch is the director of nursing for a 400-bed nonprofit hospital in the Southwest. Susan Smith joined the hospital as a staff nurse three years ago after relocating from the northeast. She is 30 years old and has been a staff nurse since graduating from a two-year college nursing program 10 years ago. She is married to a lawyer, and they have two children, ages 6 and 8. The hospital’s inpatient census has been extremely high because of another hospital’s closing. The tension on the nursing floors has been running pretty high because of time pressures to discharge patients early, lack of professional staff, and an upcoming accreditation visit from The Joint Commission. Because of time restraints, Jane was unable to complete the staff’s annual performance evaluations. However, all nurses received a 5 percent pay increase. With this increase, the hospital staff is now the highest paid as compared with other hospitals within the region. Jane believes the higher pay compensates the nursing staff for their increased workload and related stress levels. Until recently, Jane had been pleased with Susan’s performance. Susan had demonstrated her willingness to work hard and had made very few, if any, patient-care errors. However, over the past three months, Jane has noticed that Susan is not performing at her same productivity level and appears to argue frequently with the treating physicians and other nurses about the patients’ treatment plans. Jane frequently hears Susan complaining that “no one listens to me,” “no one wants to hear my opinion,” and “they don’t pay me enough to do this job.” Susan was once a highly motivated, productive member of the nursing staff. One of the reasons that the cost of drug prices are higher in the United States, is that the U.S. allows drug companies to set their own price for medications. In other countries, a delegated body negotiates the price, and it is loosely based upon the benefit of the drug to the patient (Kesselheim, Avorn, & Sarpatwari, 2016).

With the prevalence of chronic diseases, the Centers for Medicare and Medicaid Services (CMS) cite part of the reasoning for high drug prices is due partially to specialty drugs (Abramowitz & Cobaugh, 2016). Do you feel that with the high number of baby boomers approaching the age of needing more medical needs, big pharma companies see a supply and demand-type situation to raise the cost of drugs?

According to the National Sleep Foundation (2013), about 30–40% of the general population reports some level of insomnia during their lives, and 10–15% experience significant, chronic insomnia. For these individuals, medications to help induce and sustain sleep may be helpful. On the other hand, sleep aids pose potential concerns, namely abuse. Some people exceed recommended doses, and some continue taking medications even after symptoms are no longer present. Others obtain medications under false pretenses, which is one form of malingering. Malingering occurs when clients make up or exaggerate symptoms for some personal gain. Although mental health professionals may not be directly implicated in the client’s deceit, their unique position to receive more accurate and honest information than other medical professionals presents ethical concerns. What is the mental health professional’s role in these instances? In which instances would it be appropriate to break confidentiality due to a concern of malingering? How could the malingering potentiality be planned for and avoided?
