Read carefully the following case because it is a bit more complex than the first one. For this activity, you can pretend that you will work with either the family, with the couple, or just with one of the family members (pick one).

 

Our potential clients are Mr. and Mrs. Chang and their children. The case study describes Mr. and Mrs. Chang as a Chinese-born couple who met in China, married also in China in their early twenties, and came to United States when Mr. Chang was promoted in an international electronic company. Their youngest child is 6 months old, and Mrs. Chang is recovering physically and hormonally from her pregnancy while also supervising their other children and maintaining the household. The oldest child called Li An was born and raised in China and moved to U.S. when he was 10 years old. He is seventeen years old at the date of the first interview and he speaks fluently both English and Chinese. The second child was born just one year after Mr. and Mrs. Chang arrived to U.S. and was named Nicole. Per her mother’s report, Nicole is seven years old and she only wants to speak English although Mrs. Chang only speaks Chinese at home and her English is “not very good.”

According to Mrs. Chang, she and her husband are in a constant battle with the seven-year-old Nicole as she fought every rule and requirement imposed. She had become very stubborn and defiant in school, and her peers were rejecting her. Nicole’s mother reported having problems saying “no” to Nicole and follow through on negative consequences. Mrs. Chang also reported that Mr. Chang was very little involved with her daughter. Actually, her daughter hardly speaks to him when he is at home. and Mr. Chang reported that he does not have many things to say to her either. She has a very good relationship with her older brother and hardly acknowledges her 6-months-old brother. In addition, Mr. Chang recognized that he and his wife were having marital problems since some years ago. Although he did not want to have another child, they thought a new child would improve their relationship. However, since the birth of the last child, Nicole’s behavior got even worse affecting also the marital relationship until Mrs. Chang asked her husband to leave their bedroom and sleep in the room for guests. Mr. Chang reported that he is very angry with his wife and very ashamed with this situation while Mrs. Chang reported feeling isolated since her family, who moved from China some years ago, is aware of the family situation and blames her for it. Mrs. Chang requested the services of couple and family therapy because she wants to help her daughter but also because she does not want to dishonor her family by filing for divorce.

The couple’s main concern is their daughter’s behavior that is described as “disrespectful, defiant, and problematic” not only in school but also at home. Mr. Chang reports that she has an “attitude” that makes his wife “upset.” Mr. Chang also complains that his wife lets his daughter get her way due to her constant tantrums. Despite his attempts to discipline and ground her, Mrs. Chang always intercedes for their daughter and undermines his authority. This situation has caused an increasing tension between the couple. Regarding the oldest child, Mrs. Chang stated that Mr. Chang is very proud of him because their son is a very respectful and a very bright student. However, she reported that he does not have a close relationship with Mr. Chang. Although Mr. Chang also reported that he is very proud of his oldest son, he confirmed that they do not talk much or do things together.

As the interview continues and a genogram* is developed, Mrs. Chang reports having an abortion in China two years before arriving in U.S. Mrs. Chang stated that she was pressured by her family in law to have an abortion since the fetus was a baby girl but Mrs. Chang refused. However, Mrs. Chang was seven-months pregnant when local officials abducted her and forced to abort because her husband failed to pay more than $ 6,000 in fines to family-planning officials in order to have a second child. Mrs. Chang begged her husband to find a job outside China because she was terrified that officials might force her to undergo a sterilization to prevent a new pregnancy.

 

*Genogram may be developed during the first session with the family. The purpose of it is to gather more information about the family. A genogram is defined as a pictorial representation of a client’s family and relationships along with medical history. It allows the therapist to visualize hereditary patterns and psychological factors. In addition, it may help to identify recurrent patterns of behaviors (Nichols, 2013).
