1.  In the first case history (page C-11) would the single cause accident theory have been applicable?  Yes or no.
2.  In the second case history (C-12) which theory(s) would apply to the analysis and preventive measures?
3. How would this document (and videos) help employees understand the theories associated with accident causation?
