1. This chapter 11, examined the major characteristics of certain US population groups that face challenges and barriers in accessing health care services. These population groups are racial/ethnic minorities, children and women, those living in rural areas, the homeless, the migrants, the mentally ill, and those with HIV/AIDS. The gaps that currently exist between these population groups and the rest of the population indicate that the nation must make significant efforts to address the unique health concerns of US subpopulations. What reactions do you have to the ideas they presented? 1. I live in Queens, NY and from what I have seen in my neighborhood, most clinics do not provide quality care to the less fortunate patients seeking care. It is my opinion that providers see these patients just to get the reimbursement. I say this because many times the providers shift out like a cycle. One day you see a doctor and the next week for a follow up there is a new doctor who do not know the patient history. A lot of the clinics in my area have not converted to EMR system and their filing system is not organized therefore, when the patient returns for that follow up visit their is usually a new chart is started. I like others are fortunate enough to have health insurance but for those who do not I think the health care system is failing them. What reactions do you have to the ideas they presented? 1. Well, for starters, if the health care system doesn’t change, i don’t believe that the US is going to be doing very well. We are bias and don’t want to make the change that could potentially help every one of us. We hear “socialist ideas” and there is an uprising about the negative effects. On the contrary i hear that only in theory, do socialist ideas work, but i believe, for the better of humanity and the one and only planet we have , we need to make a significant change in how we run things. For starters, we need a universal health plan thats available to all, not just immigrants or minorities, or even just the privileged, it needs to be available to all. In out economic and health care crisis we’re in, its like everyone is for themselves, there is no “band of brothers” mentality. I read something that was about a man spending money to bury his car and money along side him. It ended up being a title just for readers and it was about not donating organs and leaving them to rot compared to saving countless lives. The world took an economic turn and made everything about money and corporations but tell me, how long can you hold your breath while you count all that money? Where is that money going to get you when you are 6 ft under? What reactions do you have to the ideas they presented? Jennifer Matthews: It looks like something out of Star Trek, but it’s really just a new device

to make communicating with hospital patients a little easier. This voice-activated system

allows patients to bypass secretaries and talk directly with their nurse. Retty Casey: The

improvement in communication is absolutely tremendous. Jennifer Matthews: Another

wireless device allows hospitals to quickly locate valuable equipment. Employees simply

type the in what they need and the computer shows them where to find it. Retty Casey: We

can program perimeters of the building, so if something were to walk out the front door of

the hospital, it would alarm, similar to if you were to leave a department store with a leather

coat. It’s the exact same technology. Jennifer Matthews: To prevent medication errors,

hospitals are also using smart IV pumps. Alarms sound if someone exceeds the

programmed dose. Surgeons often use cameras in laparoscopic surgeries, but now,

doctors are also using them to perform open procedures. There’s less chance of infection

because surgeons don’t have to peer over the body. This new system attaches cameras to

overhead lights. All this technology cost more than $8 million, but doctors hope to save

money by reducing errors and ultimately improving care. I am Jennifer Matthews reporting.

Andrew Holtz: You’re watching this video on your computer or mobile phone or PDA. So you probably know all about Twitter right. It’s all the rage among celebrities, politicians and businesses trying to extend their

communication reach. But there’s an industry that’s lagging behind. I’ll give you a hint it’s the industry that’s still trying to figure out how to use email. And it relies on faxing, paperwork. How many faxes do you send these days? That industry that’s still so 20th century is healthcare of course. The chief information security officer at organ health and

science university Dr. Ronald Marcum says, one reason for the reluctant to jump into social networking technology is that confidentiality and patient trust must come first. Dr. Ronald Marcum: Its very nature social networking is a very public forum. And it is an inclusive media relevant exclusive media. Now, from some business prospective that maybe fine but if you’re a patient and there’s confidentiality involve that may not be fine. Andrew Holtz: While many doctors are not using social media professionally. One New York, pediatrician is taking full advantage of Twitter for her business. Dr. Joanna Dolgoff: Twitter is a great way to get your message out to a large number of people, all across the world, with very little effort. Less than 140 characters and you can be giving advice to people from different countries. It’s been a wonderful way for me to grow my business and social networks. Andrew Holtz: But Dr. Joanna Dolgoff is quick to say she only gives very general advice on her Twitter site. It’s not for direct doctor patient communication. She say’s her colleagues won’t even go that far. Dr. Joanna Dolgoff: The doctor’s that I know around tell me they want nothing to do with Twitter. “I can’t believe you’re doing that, it takes up so much time”.

None of them want to be on Twitter. I’ve invited many of them to come on Twitter, to learn about it, so far the doctors I know really have no interest. I believe it’s because for them they don’t see the local uses. You know, they would want to build their own practice. They’re not interested n being like the National childhood obesity expert, as I am. They are

more interested in building their local practice. But the truth is, you can still get in touch with a lot of local people on Twitter and they don’t really see that. Andrew Holtz: Dolgoff predicts that eventually doctors will be on Twitter and Facebook. Ronald Marcum is more cautious. Dr. Ronald Marcum: Well I think that we know the full benefit or hazards of this particular technology primarily because it’s a work in progress if you will. The technology is rapidly being developed to the capabilities are rapidly being developed. And I think you have to look at those individual elements as they become available. Andrew Holtz: While doctors try to figure out the proper role for online social networking the marketing departments of a few major healthcare systems are signing on. The Mayo Clinic, Johns Hopkins and Mount Sinai Hospital all have a presence on Twitter, Facebook and U2 after all that’s where growing numbers of their patients are spending more of their time.

1.  Communication is a powerful tool in business.  If you have good communication between the employees, management and the administrative team your hospital should be successful.  At our hospitals we have a newsletter monthly for all the employees and I try to keep them informed of all our project updates and future plans for our hospital.  We have a management team meeting each morning with all the directors and our administrative team meets daily to talk about what we are working on and where we are on our projects. How important is communication with your leadership? 100 to 150 words

 

1. Do you think it is important for an organization to have positive mentoring programs? Why or why not? Provide an example of a time when having a mentor contributed to your success? 200 words

 

1. So I do think it is important for an organization to have positive mentoring programs because they help the younger people open their eyes to something bigger in the particular field. For instance, in health care they have mentors to help show the new employees around their company and see what else they have to offer at the hospital or facility they are working at. The programs are a great thing because it helps people see other things in the facility. An example I can provide of a time when a mentor contributed to my success was probably back in high school when the guidance counselors would help each of the students pick out their schedules. For me, this was a success because I was able to graduate early and continue on with my education earlier. Not only that but I was also able to help out as an administrator in the office at the school since I had taken most of my classes my freshmen through junior year. I only had to take a few classes my senior year and was able to graduate early as well. Having these mentors were a huge help when picking out my classes because they were able to give their insight on our life at school as well. Those counselors at the high school also helped us make big decisions and provide examples from their life too. What reactions do you have to the ideas they presented? 1.  Guidance counselors have been important to my college career also.  When I was in high school our counselor helped me pick out a good college to go to and helped me get signed up and accepted by them.  The two counselors were instrumental in getting a lot of people into college at our high school.  How important were the high school counselors in your school?  Did they help you?  How? 150 words

1. When I go to a new hospital the first few months I walk around and talk to people and I go to each different department to get to know people.  I talk to them but I do not make many decisions or make changes until I have open communication with the leaders and employees of the hospital.  When I first started as a CEO I made quick decisions in my new job and made enemies because I did not understand the managers or the employees and it took me a few years to get everyone on the same team so I learned from my mistakes.  I have taken my time and made decisions slower since then and I communicate with the managers to make sure they are comfortable with my decisions.  Class how does your administrative team work with your company employees? 100 to 150 words

Case Scenario 3: The quarterly reports of expenditures for all the St. Agnes Health System have been distributed. You are the Director Team for the Orthopedic Clinic, and your area has seriously overspent its budget for the third consecutive quarter. There are rumors circulating that your job depends on reducing costs for supplies, the most expensive item on the list. Other rumors suggest that all the clinic nurses fear they will lose their jobs if the poor financial report for the clinic causes the board to convert the clinic into an urgent care. Although morale has been low, you have been adequately staffed and there seems to be an increase in sick calls by everyone on the team.
