                      Community Health Needs Assessments

                      Sentara Healthcare has conducted community health needs assessments for our inpatient hospitals and outpatient surgical hospitals in the areas that we serve. The assessments provide us with a picture of the health status of the residents in our communities and provide us with information about health and health-related problems that impact health status.

                      Our assessments include a review of population characteristics such as age, educational level, and racial and ethnic composition because these factors can impact health. The assessments also look at risk factors, such as obesity and smoking, and health indicators, such as infant mortality and preventable hospitalizations. Community participation is important, so the assessment also includes input from local health departments, the school system, social services, community health centers, free clinics, local governments, and many others through a survey, focus groups and other means.

                      The needs assessment identifies numerous health issues that our communities face. Considering factors such as size and scope of the health problem, the severity and intensity of the problem, the feasibility and effectiveness of possible interventions, health disparities associated with the need, the importance the community places on addressing the need, and consistency with our mission to improve health every day, we have identified a number of priority health problems in our areas.

                      The community health needs assessments were used as the foundation for the hospital implementation strategies to address the priority needs identified. The assessments and implementation strategies have been adopted by each hospital’s governing body. A number of resources are available in the community to address these needs through community partners such as the local health departments, United Way Agencies, academic institutions, and others. Information about these resources is available from sources like 2-1-1 and Sentara.com. Together, we will work to improve the health of the communities we serve.                       Workplace Environment Assessment Continuation

                      Clearly, diagnosis is a critical aspect of healthcare. However, the ultimate purpose of a diagnosis is the development and application of a series of treatments or protocols. Isolated recognition of a health issue does little to resolve it.

                      In this module’s Discussion, you applied the Clark Healthy Workplace Inventory to diagnose potential problems with the civility of your organization. In this Portfolio Assignment, you will continue to analyze the results and apply published research to the development of a proposed treatment for any issues uncovered by the assessment.

                      To Prepare:

                      · Review the Resources and examine the Clark Healthy Workplace Inventory, found on page 20 of Clark (2015).

                      · Review the Work Environment Assessment Template.

                      · Reflect on the output of your Discussion post regarding your evaluation of workplace civility and the feedback received from colleagues.

                      · Select and review one or more of the following articles found in the Resources:

                      This is a Collaborative Learning Community (CLC) assignment.

                      As a group, identify a research or evidence-based article published within the last 5 years that focuses comprehensively on a specific intervention or new treatment tool for the management of diabetes in adults or children. The article must be relevant to nursing practice.

                      Create a 10-15 slide PowerPoint presentation on the study’s findings and how they can be used by nurses as an intervention. Include speaker notes for each slide and additional slides for the title page and references.

                      Include the following:

                      Describe the intervention or treatment tool and the specific patient population used in the study.
                      Summarize the main idea of the research findings for a specific patient population. The research presented must include clinical findings that are current, thorough, and relevant to diabetes and nursing practice.
                      Provide a descriptive and reflective discussion of how the new tool or intervention can be integrated into nursing practice. Provide evidence to support your discussion.
                      Explain why psychological, cultural, and spiritual aspects are important to consider for a patient who has been diagnosed with diabetes. Describe how support can be offered in these respective areas as part of a plan of care for the patient. Provide examples.
                      You are required to cite to a minimum of two sources to complete this assignment. Sources must be published within the last 5 years and appropriate for the assignment criteria and relevant to nursing practice.
