Client History Report Draft

For this assignment, you will write the first four sections of the psychological assessment report that will be completed in Week Five. Taking on the role of a psychological evaluator, you will formulate vitally important methodological and theoretical assessment conceptualizations of a client during the initial phases of the assessment process. You will apply your conceptualizations to develop a clear understanding of the presenting concerns. In the initial sections of your psychological assessment report, you will summarize results obtained from examining the following information pertaining to your client: identifying information, reason for referral, presenting concerns, and relevant personal history. The content of this section must demonstrate an ethically and professionally responsible rationale for the use of psychological assessment tools. View the complete instructions for the Final Paper in the link within Week Five of your online course or the “Components of Course Evaluation” section of this guide. To complete this assignment, you will choose one of the following clients.

This week, your assignment must cover the following sections of your psychological assessment report and include the headings as listed:

I. Identifying information
Within this section, you will record basic information on your client including the person’s name, sex, gender, ethnicity, sexual orientation, age, handedness, and occupation or grade level. For the purposes of this assignment, you are free to create any relevant demographic information that is not explicitly stated in the case scenario. All information you create must be consistent with the information provided and any conclusions you draw in subsequent sections of your paper.

II. Reason for referral
Within this section describe the referral source and the purpose of the assessment. The information you provide in this section must justify the decision to conduct a formal psychological assessment based and must model ethically and professionally responsible assessment practices.

III. Current Symptoms/Presenting Concerns
The information in this section of the report would typically come from an interview with the client and family (if applicable, e.g., if the client is a child or person with suspected dementia).  You must use the information provided in case history to identify the most salient information that belongs in this section. Choose information to include in this section based on the consistency with the reason for referral and purpose for testing. Here is where you will apply your methodological and theoretical assessment formulations of the client that will justify the decision to conduct a psychological evaluation on this client.

IV. Psychosocial History (complete each of the sections below based on the information in the case you selected)

  • Educational history
  • Occupational history
  • Medical history (including substance use/abuse)
  • Psychiatric history
  • Social history

Your assignment must be of sufficient depth and detail to support and inform your eventual diagnostic impressions. A cursory or surface level review of the client’s presenting problem and history will unlikely provide enough information for your diagnostic impression. The assignment must also include a page with relevant references based on your initial impressions of possible diagnostic considerations from the history that you have reviewed.

The Assignment:

  • Must be at least 2.5 to 3 single-spaced pages in length (not including title and reference pages).
  • Must include a title page with the following:
    • Title of paper
    • Student’s name
    • Course name and number
    • Instructor’s name
    • Date submitted
  • Must use at least 2 scholarly sources, 1 of which must be an original peer-reviewed research article, in addition to the course text.
  • Must document all sources in APA style as outlined in the Ashford Writing Center.
  • Must include a separate title page and reference page that is formatted according to APA style as outlined in the Ashford Writing Center (Links to an external site.)Links to an external site..

Part III: SPSS Data Entry and Analysis

The steps will be the same in Part III as the ones you have been practicing in Part I of the assignment; the only difference is that you are now responsible for creating the data file as well. Remember to do the following:

· Name and define your variables under the “Variable View,” then return to the “Data View” to enter the data

· Paste all SPSS output and graphs into your homework file at the appropriate place.

Part III: Questions 1-3

· Research scenario: A cognitive psychologist is conducting a study to look at the effects of recent substance use on reaction time. Using a sample of volunteers from the intake ward of a local residential rehabilitation facility, she gathers data on the participants’ recent substance use and measures their reaction time in hundredths of a second using a variation of the “red light, green light” test ( https://faculty.washington.edu/chudler/java/redgreen.html ) . She consequently has two variables, one for the type of substance used and one for reaction time. Follow the directions below, and answer the questions that follow:

· Open a new SPSS data file.

· Create two new variables called “SubAbuse” and “ReacTime” in the SPSS file. Use “Variable View” to create these variables.

· The ReacTime variable should be coded as “Scale” within the “Measure” column. The SubAbuse variable should be coded as “Nominal.” For SubAbuse, under the “Values” column, define 1 as “Alcohol”, 2 as “Opioid”, 3 as “Marijuana”, and 4 as “Cocaine.” (For a review on this process, see the SPSS Intro presentation in Module 1.)

· Return to the “Data View” and enter the data shown here:

1) Create a bar chart for the SubAbuse variable in SPSS. Use the “Summaries for Groups of Cases” method.
Answer- Bar chart: SubAbuse (paste Figure in this cell)

  2) Create a histogram for the ReacTime variable.
Answer- Histogram: ReacTime: (paste Figure in this cell)

  3) Though bar charts and histograms look similar, they are two different types of graphs. Explain why it is appropriate to create a bar chart for the substance abuse variable, and a histogram for the reaction time variable. Pay special attention to the X axis (horizontal axis) of each graph. If necessary, review the “How to Build a Graph” section in Chapter 3 of Nolan and Heinzen.

(2012). Please also read the Rizvi, et al. (2013), “An Overview of Dialectical Behavior Therapy for Professional Psychologists,” Harned, et al.  (2013), “Treatment Preference Among Suicidal and Self-Injuring Women with Borderline Personality Disorder and PTSD,” Miller (2006), “Telehealth Issues in Consulting Psychology Practice,”  and Luxton, et al. (2011), “mHealth for Mental Health: Integrating Smartphone Technology in Behavioral Healthcare” articles.

Assess the evidence-based practices implemented in this case study. In your paper, include the following.

· Explain the connection between each theoretical orientation used by Dr. Bank’s and the interventions utilized in the case.

· Describe the concept of dialectical behavior therapy, being sure to include the six main points of this type of treatment.

· Explain Dr. Banks’s primary goal during the pre-treatment stage and how Dr. Banks related this to Karen in her initial therapy sessions.

· Describe the two formats that Dr. Banks told Karen would be part of her treatment program.

· Describe the focus of the second and third stages of treatment.

· Assume the role of a consulting clinical or counseling psychologist on this case, and recommend at least one technology-based e-therapy tool that would be useful. Explain liability issues related to delivering e-therapy consultation, supporting your response with information from the Miller (2006), “Telehealth Issues in Consulting Psychology Practice” article.

· Evaluate the effectiveness of the treatment interventions implemented by Dr. Banks supporting your statements with information from the case and two to three peer-reviewed articles from the Ashford University Library, in addition to those required for this week.

· Recommend three additional treatment interventions that would be appropriate in this case. Use information from the Sneed, Fertuck, Kanellopoulos, and Culang-Reinlieb (2012), “Borderline Personality Disorder” article to help support your recommendations. Justify your selections with information from the case.

The Case Analysis – Treatment Format

Must document all sources in APA style as outlined in the Ashford Writing Center.

PSY650 Week Four Treatment Plan

Behaviorally Defined Symptoms: Karen displays extreme emotional reactions at the hint of perceived abandonment in a relationship. There is a history of unstable and intense interpersonal relationships, impulsive behaviors, and recurrent suicidal gestures.

Diagnostic Impression: Borderline Personality Disorder

Long-Term Goal: Terminate self-harming behaviors (substance abuse, cutting, and suicidal behaviors).

Short-Term Goal: Reduce the frequency of maladaptive behaviors, thoughts, and feelings.

Intervention 1: Dr. Banks will outline the process of Dialectical Behavioral Therapy.

Intervention 2: Karen will commit to attending group behavioral skills training and individual psychotherapy.

Intervention 3: Karen will participate in imaginal exposure to trauma, until the memories no longer cause marked distressed.

For additional information regarding Karen’s case history and the outcome of the treatment interventions, please see Dr. Bank’s session notes under Case 15 in Gorenstein and Comer’s (2015), Case Studies in Abnormal Psychology
