Consider the population that you hope to work with as a counselor, such as at-risk youth, or people with anxiety. Use the Mental Measurements Yearbook (MMY) and the Alexander Street Videos (AVON in the Walden Library) to find out more about these topics and appropriate assessments.

This week, your assignment must cover the following sections of your psychological assessment report and include the headings as listed:

I. Identifying information
Within this section, you will record basic information on your client including the person’s name, sex, gender, ethnicity, sexual orientation, age, handedness, and occupation or grade level. For the purposes of this assignment, you are free to create any relevant demographic information that is not explicitly stated in the case scenario. All information you create must be consistent with the information provided and any conclusions you draw in subsequent sections of your paper.

II. Reason for referral
Within this section describe the referral source and the purpose of the assessment. The information you provide in this section must justify the decision to conduct a formal psychological assessment based and must model ethically and professionally responsible assessment practices.

III. Current Symptoms/Presenting Concerns
The information in this section of the report would typically come from an interview with the client and family (if applicable, e.g., if the client is a child or person with suspected dementia).  You must use the information provided in case history to identify the most salient information that belongs in this section. Choose information to include in this section based on the consistency with the reason for referral and purpose for testing. Here is where you will apply your methodological and theoretical assessment formulations of the client that will justify the decision to conduct a psychological evaluation on this client.

IV. Psychosocial History (complete each of the sections below based on the information in the case you selected)

  • Educational history
  • Occupational history
  • Medical history (including substance use/abuse)
  • Psychiatric history
  • Social history

Your assignment must be of sufficient depth and detail to support and inform your eventual diagnostic impressions. A cursory or surface level review of the client’s presenting problem and history will unlikely provide enough information for your diagnostic impression. The assignment must also include a page with relevant references based on your initial impressions of possible diagnostic considerations from the history that you have reviewed.

    Company X is a rapidly growing automobile sensor-manufacturing firm headquartered in small-town Gaylesburg, Ohio. Over the last two decades, the company has progressed from a small family owned and operated company with regional prominence to a leading national firm. For over two decades, the company thrived with the support of a hometown talent pool that was 99% Caucasian; 1% African American, and 98% male; 2% female in composition. When the founder passed leadership on to his eldest son, the new CEO actualized his vision to expand the company and increase its market share. As a result, the company struggled with the development of a more culturally diverse workforce.
    Presently, the CEO wants to preserve the company’s family culture in the face of large-scale growth and movement into several urban hubs. Presenting Problem

Client states, “I recently lost my job and feel hopeless. I can’t sleep and don’t feel like eating.” Client also reports she has lost 10 pounds during the last two months. Client states that she is a solo parent and is worried about becoming homeless. Client states, “I worry all the time. I can’t get my brain to shut off. My husband is in the military and currently serving in an overseas combat zone for the next eight months. I worry about him all the time.”



Behavioral Observations

Client arrived 30 minutes early for her appointment. Client stated that she had never been in counseling before. Client depressed and anxious, as evidenced by shaking hands and tearfulness as she filled out her intake paperwork. Ana made little eye contact as she described what brought her into treatment. Client speech was halting. Client affect flat. Client appeared willing to commit to eight sessions of treatment authorized by her insurance company.

General Background

Client is a 24-year-old first-generation immigrant from Guatemala. Ana was furloughed from her job as a loan officer at local bank three months ago. Client reported that she was from a wealthy family in Guatemala, but does not want to ask for help. Client speaks fluent Spanish.

Client has completed one year of college with a major in business. Client states that she left college after her son was born as she found it difficult to manage a baby, college, and a full-time job.

Family Background

Client is the middle of four siblings. Client has two older brothers and one younger sister. Client’s parents have been married for 27 years. Client states that she has had a “close” relationship with her family, although she states that her father is a “heavy drinker.” Client states that all her brothers and sisters have graduated from college and have professional careers. Client states that her father is a banker and her mother is an educator. Client states that she has not seen her family for 1 year. Client has a 1-year-old son and states that she is sometimes “overwhelmed” by raising him alone.
