For the second written assignment of the course, you will continue in the design of your proposed model program by demonstrating your understanding of your selected population’s challenges, which negatively impact this group’s health and well-being. Based on this week’s research, conduct an assessment of the barriers, limitations, and other distinguishing features, as they exist within your community.
Prepare a recap of the model program for your community that you originally shared in the week 2 written assignment. As stated in the Week Two directions, changes to the potential program can be made as you research and develop the focus of the program.
Analyze and discuss at least three critical barriers that impact the health and well-being of your chosen group; one must be a micro-level (individual) barrier that is financial, one must be a macro-level (community/state) barrier that relates to access and funding for care, and the third barrier may be one of your choosing.
Discuss at least one proposed solution for each barrier. Your solution for the micro barrier must include an analysis of various potential funding options (both independent and integrated). Your solution for the macro barrier must include an analysis of financing resources for health care.
Research and analyze the regulatory, legal, ethical, and accreditation requirements and issues for the service(s) offered in your proposed program. Discuss how each will impact the management of the program.
Your assignment should be a minimum of three pages in length (excluding title and reference pages) and should include a minimum of three scholarly sources cited according to APA guidelines as outlined in the Ashford Writing Center. Please note: All assignments in this course are progressive; therefore you should use the same population selected in your Week Two assignment. The Week Two assignment’s contents do not need to be re-submitted with this assignment.

Informed consent is an integral part of the counseling process, regardless of setting, and indicates approval by a client to participate in counseling. In order for the consent to be informed, the client must first achieve a clear understanding of the relevant facts, risks and benefits, and available alternatives involved. This assignment will help counselors understand what needs to be included in an informed consent form as well as how this form can be one of many essential risk management tools counselors can use to ensure adherence to ethical codes and practice procedures.

Tasks:

In a 4- to 5-page paper, respond to the following: Using current ethical and legal standards, specifically the 2014 ACA Code of Ethics (ACA, 2014) and legal requirements of your state, territory, or district, develop an informed consent form that you could use in actual practice (either private or community based). Customize the form to work with an adult client who comes to you for assistance with suicidal ideation, a history of depression, and an extensive history of seeking professional counseling. Create your own form from scratch. Do not cut and paste an existing form or use one from another organization or that you found on the internet.In addition to the form, summarize when during the counseling process you would use this form and the specific ethical and legal standards you addressed in the creation of the form. Clearly explain why you included each section or item in the document. Be sure to address multicultural and ethical considerations you need to consider when developing and using this form, collaboration with other professionals, and relevant laws in your state (e.g., mandated reporting laws for the state in which you practice).Finally, discuss how this informed consent document fits into your personal model for ethical practice. In addition, what other risk management strategies should you take as a professional counselor to ensure ethical practice?

Your final product will be in a Microsoft Word document and be approximately 4–5 pages in length (including the consent form). The informed consent should be approximately 2–3 pages . Your form and supplemental information should be organized in a clear, concise, and organized manner; and display accurate spelling, grammar, and punctuation.

Portfolio Requirement

As a student in the CMHC program, you are required to create a portfolio documenting both your academic work and professional activities completed during the program. The informed consent form is an appropriate piece of evidence to add to your portfolio.

Consider yourself, your parents and grandparents; which theory of aging from chapter 25 best describes what your elders are experiencing and what you expect of yourself and the others as you age? What behaviors justify putting yourself and/or them in those categories of aging lifestyles?

“Teaching Module”

Create a teaching module, a PowerPoint or a video that teaches 20 course concepts on the theme of your choice. Post a PowerPoint here. Post the video on YouTube (send me email with URL so I can grade it). Begin PowerPoint with an introductory slide that includes your name, a title that represents the theme and objective of your presentation, and information about the course (5 points). The next slide should be a message to your audience, so have your audience in mind as you create your presentation (5 points): are they Eagle Scouts, PTA mothers, parole or police officers, volunteers who plan to speak to. Psychologists are aware of how much people in the world need information, and you can bring it to some of them. Have 20 slides that cover information about 20 Learning Objectives from the course (80 points). You have freedom here to choose the topics you find most relevant to life in the outside world. End with a slide or two with conclusions and implications of the information for your readers in the outside world. If you are doing a video, include all the information that is required for the PowerPoint Presentation. Remember to relate your slides to the objectives posted in each module of the course over the chapters covered in that unit. You can pick the chapters and objectives that fit your theme. Your last two slides should include a summary (5 points) and a final note to your audience about your hopes for how they can use this information (5 points.
