Scenario
As the quality control supervisor at Aero Coast, you make sure that the structural repairs on wide body passenger and freight aircraft are completed in compliance with all federal regulations. Aero Coast specializes in re-fitting aircraft and they have developed quite a reputation for innovative structural designs. In addition, Aero Coast is known for their attention to detail and finishing projects on time.
You and the team have just completed a structural re-design to the interior of a plane that will be used to ship mini-widgets overseas. The re-design focused on customized racking systems to hold the sensitive mini-widgets in place on the long flight. About four hours after the plane has left, you get a text from the pilot. The plane has been grounded in the northeast because of “improper aircraft configuration.” The lens covers on the emergency lighting system are not FAA specified.
After a few calls, you find a line mechanic that can make the necessary repairs, but he can’t get there for about 24 hours. You call the client and deliver the bad news–the flight will be delayed by a day. The client, Global Widget Freight, has been with Aero Coast for about three years, and you have a solid business relationship. They account for about 17% of your annual revenue, but they’ve just been bought out.
You have some work to do to keep people happy and to make sure this type of problem does not happen again.

The DMAIC process streamlines and organizes this process into phases: Define, Measure, Analyze, Improve, and Control.

The “A” in DMAIC stands for Analyze. The “Analyze” phase involves the Root Cause Analysis (RCA) of any problem, issue, or situation. Important note: The root cause is not the problem itself. The root cause creates the problem. When searching for the root cause, asking “Why” is an important step. In fact, there is a specific tool for this – a Why-Why Diagram. Why-Why Diagrams look like a tree. The tree has roots – see the connection?

As an example (and this is a true story), quite a few cars stopped running after drivers purchased fuel at several Sam’s and Costco’s. The fuel was traced back to a gasoline tank that mistakenly contained diesel fuel instead of gas. But this wasn’t the root cause. Upon further investigation and digging deeper, the supplier put diesel fuel into the wrong tanker before delivering to Sam’s and Costco. The root cause was either insufficient training or a poor system of checks and balances – or perhaps a combination of both. If we start asking the “why” questions, we can usually arrive at the root cause.

Six Sigma teams often have brainstorming sessions to toss around ideas when starting an RCA. For this discussion, you are the Champion of a Six Sigma Team, so you will initiate a Root Cause Analysis.

  • Select one of the problems or situations listed below. As noted, you may use a “real” problem if you prefer.
  • Meet with a friend, family member, or coworker and conduct a brainstorming session to determine possible root causes.
  • List five possible root causes or reasons why this problem or situation exists.
  • Rank these five possible root causes in order of importance or probability.

  • Justify your decision by describing the benefit(s) of dealing with or correcting this first or main root cause.

Two models of grieving—the Kubler-Ross and Westburg models—identify stages through which an individual progresses in response to the death of a loved one. Understanding the various ways individuals cope with grief helps you to anticipate their responses and to assist them in managing their grief. Addressing the needs of grieving family members can diminish your personal emotional, mental, and physical resources. In addition to developing strategies to assist grieving individuals in crisis, you must develop strategies that support self-care.

In this Assignment, you apply a grieving model to work with families in a hospice environment and suggest strategies for self-care.

Submit a 2- to 3- APA page paper in which you:

  • Explain how you, as a social worker, might apply the grieving model of Kubler- Ross to your work with families in a hospice environment.
