You are the new chief of the Greenfield Police Department. After 30 years of iron-fisted control, Chief Slaughter has retired. Slaughter believed in the military model of police management and a traditional crime-fighting policing strategy. He was fully entrenched in the war on crime and ran his department like an army unit. His book of rules and regulations was a foot thick, and he demanded absolute compliance. Decisions were made in the chief’s office and passed down to the officers through layers of captains, lieutenants, and sergeants. At Chief Slaughter’s retirement ceremony, the mayor slaps you on the back and says, “You’ve got some big shoes to fill, son. That guy knew how to fight crime, and his officers never stepped out of line. Our crime rate was below the national average every year he was here.” The City Council presents Slaughter the Meritorious Service Award for 30 years of crime fighting. As a student of police history you realize that most police departments battle complex social problems and seldom march off to war. You know that crime rates are minimally influenced by crime fighting and are a poor indication of policing success. You also know that traditional organizational structures and policing strategies are slow to change and often are out of sync with one another. Most of your questions to the captains about department operations have generated the same response: “Because that’s the way we’ve always done it. If it ain’t broke, why fix it?”

You decide to visit with members of the community. A homeowner tells you that Chief Slaughter’s officers do a great job of patrolling her neighborhood, but she’s worried about the future impact of the deteriorating apartment complex across the street. She realizes it’s not a police problem. The manager of a senior citizens’ residence tells you that there hasn’t been a crime reported in their neighborhood in over a year, but the residents are afraid to go out at night. He thinks it’s the rumors that spread from crime reports on the television news. The business owners in the shopping center complain that customers are being driven away by kids skateboarding in the parking lot. They understand that the police have more urgent crime problems to fight. The high school principal praises the police department’s stringent traffic enforcement before and after school. He wishes he could resolve the growing truancy problem as efficiently as the police handle traffic. None of the people you talk to is personally acquainted with a Greenfield police officer. It appears the Greenfield Police Department is trapped in the reform era. They rely on preventive patrolling and rapid response as their primary policing strategies and seldom interact with the community. You review their mission statement and find it emphasizes the professional model of crime fighting.

APN roles are broken down into more generalized areas of Certified Registered Nurse Anesthetist, Clinical Nurse

Specialist, Nurse Practitioner, and Certified Nurse Midwife. The U.S. have their own individual nurse practice acts that discuss regulations for APN’s to include education, certification/licensure, and scope of practice (Watson & Hillman, 2010). Even though APN’s scope of practice may be different in each state APN’s can  universally can diagnose where specialty nurses can still only rely on nursing diagnosis. I do believe APN roles will became more standardized as the need for APN’s increases. According to Lathrop and Hodnicki (2014) if the Conesus Model for APRN Regulation were to be used then standardization for all APRN licensure, accreditation, certification, and education would be done through the legislation.Nurse Practitioners currently receive their credentialing by five different bodies dependent on the area of interest. These are the American Academy of Nurse Practitioners Certification Program, American Nurses Credentialing Center, American Association of Critical-Care Nurses, and Orthopaedic Nurses Certification Board (Watson & Hillman, 2010).

According to Hamric, Hanson, Tracy, and O’Grady, (2014) specialty nursing practice is nursing focused on a specific field where as “advanced practice nursing reflects concentrated knowledge in a specialty that offers the opportunity for expanded  and autonomous practice based on broader practical and theoretical knowledge base” (p. 113). Specialty nursing practices obtain their training through on the job training and/or their health care institutions through certification versus advanced practice nursing (APN) obtain their training through a formal educational graduate level program (Hamric et al., 2014). Watson & Hillman (2010) reported the required education level of an APN was going to be doctorate by 2015. Here we are in 2017 and it is still that of a Masters level.

I  believe APN’s will be required to have a Doctorate level of education in the near future due to the evolving health care needs of our population as well as due the increase in legal liability. Since the passage of The Affordable Care Act the need for Advanced Practice Registered Nurse with a Doctor of Nursing Practice has increased due to preventive health focus (Lathrop & Hodnicki, 2014). As we are all aware APN’s have historically been focused on primary care which puts us as the leader in this shift in health care. I believe this because he start as registered nurses and provided that direct patient care only as nurses we follow the treatment plan developed by the primary care provider and now as APN’s we can develop the treatment plan.

We take it very seriously because we think it’s very important. Academic and professional integrity for me are one and the same. I don’t think you can have one without the other. Nurses are a very trusted profession, and we are expected to have integrity. Integrity in the workplace of course means being honest, doing the best we can to provide the best possible care. The same thing applies in an academic setting. A student that has good academic integrity does their own work. And while you may collaborate in terms of discussion and sharing of ideas, you don’t share work. FEMALE SPEAKER: Academic integrity goes hand in hand with professional integrity. We wouldn’t do something in the professional realm in terms of taking notes from others or taking copyrighted information from others and making it our own. We shouldn’t be doing that in the academic setting either. FEMALE SPEAKER: Make sure you know how to properly cite if you are going to use a direct quote from somewhere, using the proper citation. And as a graduate student, I would actually recommend that you learn to use your own words because you are going to be an expert, and it’s much better to read from multiple sources and use your words to promote what idea you’re putting forth and give credit for the idea, but not the quote. FEMALE SPEAKER: A lot of times we paraphrase things. And sometimes we don’t take into context that perhaps we should cite something. Ask the question. There’s nothing wrong in asking. I would rather have students ask the question in terms of, do you think this is the approach that I should take? FEMALE SPEAKER: It is a huge component of what we do as nurses to be ethical and have integrity, knowing that we are doing the best for our patients every single day. FEMALE SPEAKER: You take an oath to do what’s right, to do what’s good, and to do no harm to your patients. And so you must have integrity to be a nurse. FEMALE SPEAKER: Honesty, trust, integrity– those are the essence of a nurse that anybody– when they think about nursing, that’s what they think about.
