6.    Include an introductory paragraph, purpose statement, and a conclusion.
7.    Include level 1 headings to organize the paper.
8.    Write the paper in third person, not first person (meaning do not use we or I) and in a scholarly manner. To clarify: I, we, you, me, our may not be used. In addition, describing yourself as the researcher or the author should not be used.
9.    Include a minimum of two (2) professional peer-reviewed scholarly journal references to support the paper.
10.    APA format is required (attention to spelling/grammar, a title page, a reference page, and in-text citations).

The purpose of this assignment is to design an age appropriate health maintenance and screening plan for one adult primary care client.

1. Complete the Primary Health Promotion Assessment Form (Dunphy text 5th edition, Chapter 3, Box 3.4, pages 36 – 39) for one patient you see in practice during Week 1 or Week 2 of the course. If you have not begun your practicum, you may complete this assignment on a friend or relative.
2. The document should have a title page in APA format.
3. The patient should be between the ages of 45 and 65.
4. The patient may be male or female.
5. Based on the data collected in the Primary Health Promotion Assessment Form, you should formulate health maintenance and screening plan based on the U.S. Preventive Services Task Force.
