PURPOSE: This assignment is to orient the students to the community used for the community health nursing clinical. (16 hours) GROUP NAMES: Dainelis, Camelia, Adidley, Arisbel

 

The assessment is to be done individually and may require time other than the assigned clinical day for completion. Be prepared to present your findings, orally, in seminar. Date and time of presentation will be scheduled. The following guideline is to focus your assessment.

 

1. On your way to the designated community:

a. Did you go through different small communities? The different types of towns around are Wynwood, allapattah, midtown, and overtown which is where Camillus house is located. How did they differ? Living in Allapattah offers residents an urban suburban mix feel, and most residents rent their homes. Based on pictures the neighborhood looks a bit poor and the crime rates are high. In comparison to Wynwood which is the new art district which is becoming quite famous. Wynwood is an outdoor art exhibit at city scale, unrivaled in its blend of well-known names and up-and-coming artists. Though its street life first grew through Second Saturday Art Walks, the neighborhood is lively every day of the week. There is plenty to see, eat, drink, and explore. There are new modern apartments around compared to allapattah which has a lot more of older homes. Midtown is much closer to the beach area and has a lot of newer buildings around. Overtown is a part of the historic heart of Miami and it is known for its metro rails, museum, and parks.

b. Could you identify boundaries for different communities? (i.e., were there obvious separations between communities?) Compared to overtown which has the availability of transportation from the metrorails it can become a bit difficult for other communities to have this since they are not as close. Other areas such as Wynwood have more activities and food options but not as much healthcare options compared to overtown which has Jackson hospital and other healthcare facilities. Each have their different things they are known for and different boundaries.

c. What sights, smells, sounds, etc., were identified with the different communities? The sights were completely different in each community for example Wynwood is surrounded by color, lights, people, music and activities. Some of the smells in Wynwood are of food since there are a lot of restaurants around. Allapattah have much older buildings with paint that is chipped around many of the streets and buildings. Midtown is filled with new buildings and structures as well as the smell of the beach and more of pure air since it is a lot closer to the ocean.

 

2. After arrival in the designated community:

a. What evidences of a minority culture do you see? How does this affect the environment? (Are there advertising billboards, graffiti, etc., that reflect ethnic characteristics of community?)

b. Who do you see on the streets: elderly, young males, children, and women? (Indications of unemployment, constituencies, etc.)

c. What industries are represented in the community?

d. What would it be like to live in the community?

e. Is public transportation available?

f. How far to supermarkets and shopping centers?

g. What type of housing?

h. Where are “For Sale” signs?

i. Do you think people rent or own the housing?

j. What is the social-economic status of this community?

k. What recreational facilities are there in the area?

l. What kind of churches do you see in this area?

m. What potential environmental hazards did you identify?

 

3. What are the medical care facilities in this area? Doctors’ offices? Any evidence that physicians reflect the ethnic background of the population? How accessible are the medical facilities? Mount Sinai Medical Center Midtown, University of Miami Health system, Jackson Memorial Hospital, New Health community, Brave Health Corporate office, MD Urgent Care, Chistine E.Lynn Rehabilitation Center. Based on my research around Camillus House the population Race and Ethnicity is mainly Hispanic, African -American and a small percentage White. There are a variety of medical care facilities nearby the location with physicians, nurses from different ethnicity group which is really helpful and easier for the patients to communicate with them. The facilities are within 5 miles range, for example Jackson Memorial Hospital is within 14 minutes walking and 0.6 miles from Camillus House. Most of the one listed above are within that mile range or less.

 

 

 

 

4. Find a community center, Senior Citizens Center, Chamber of Commerce closest to the city you are assessing. Complete online research. What other community resources are available in the area?

5. Develop five key questions of your own that you would ask an adult living in this neighborhood to learn more about the community. Be very specific to your community.

 

 

6. What would it be like to be elderly or homeless in this community? What resources are available to meet their needs

After doing some research I was able to gather information about resources in the community that would be beneficial for the elderly as well as homeless people. In fact, one of the centers I came across was Camillus House, another one was The Salvation Army. In this Centers, they provide the population with food, water, hygiene products and anything they could give to those in needs in order to survive, as well as a place to stay the night.

 

7. What would it be like to be a young single teenage mother in this community?

Pregnant and Parenting teens often are balancing their lives and being a parent. Ensuring that adolescent parents receive adequate social and emotional, medical, and academic support is essential to the parent and the baby’s future.

It is clear that being a child of a teenage mother often entails numerous risks: low birth weight, complications of the mother’s pregnancy and delivery, and health problems associated with poor perinatal outcomes; greater risk of perinatal death; lower IQ and academic achievement later on, including a greater risk of repeating a grade; greater risk of socio-emotional problems; a greater risk of having a fatal accident before age one; and finally, a greater probability of starting one’s own family at an early age.

Since this community is a low-income community, those single teenager mother have biggest risk to have babies with malnutrition, lack of access to prenatal care, poverty and ignorance.

On the other hand, there many resources that single teenager mother can use like:

– Young Girls to Young Ladies Club Program

– Family Planning in Miami Dade County

– The Mailman Center for Child Development

 

 

8. What resources are available for them? In overtown you have many resources such as libraries which are Overtown Public Library and Dorsey Memorial Library and it offers transportation through the Metrorail’s as well as shelters such as Camillus house for the homeless or those in need. It offers medical centers such as Jackson. Some of the other resources include Homeless Assitance center, salvation army Miami adult rehab center, Miami rescue mission which offers employment and housing as well as Miami rescue woman, eyes on god homeless shelter, and many religious centers.

 

9. Try to find out what the power structure is in the community.

a. Who or what community organizations are influential in the community and how they are influential?

b. Identify potential public health problems/high risk groups. What solutions do you recommend?

  • Calculate the 5-year net sales, operating expenses, operating income, and net income of Jiranna Healthcare. Once calculations are complete, interpret the resulting data and explain the significance of the trend results.

Part II: Case Study #2

For the next part of this Performance Task, you are expected to complete a number of calculations, and then interpret the numbers to provide recommendations based on these analyses. It is essential both to show your work and calculations, and to demonstrate how these calculations support your conclusions. Be sure to explain your reasoning. You will be assessed on the accuracy of your quantitative analyses and the quality of the evidence you use to support your conclusions.

Imagine that you are an administrator at Jiranna Healthcare who has been asked to analyze cash-flow data to determine the costs and benefits of implementing a new capital project. For background, read “Capital Project Case Study, Part 1.” Then, complete the “Capital Project Case Study, Part 2” spreadsheet, which provides cash-flow data (costs and benefits) for the proposal. Download and save the Excel spreadsheet, and use the information provided to complete the following:

  • Calculate the cash inflows and outflows for each year.

You are an administrator at Jiranna Healthcare. You have been asked to conduct a budget variance analysis: analyzing performance by comparing budgeted workload, revenue, and expenses for a range of different service lines, with actual workload, revenue, and expenses for those service lines.

Analyze the data and prepare a 3- to 4-page report. In your report, address each of the five following questions. In each case, show the calculations that you conducted to answer the question. Then, explain your conclusion and how your calculations support your reasoning.

  • What was the hospital’s original profit forecast (assume away any issues with depreciation, taxes, etc.)? Halfway through the fiscal year, what is the hospital’s revised projection for FY11 profits?
  • Which inpatient service lines are over budget? Which inpatient service lines are over budget after accounting for workload increases?
  • What two actions would you recommend be taken at the mid-year point if you oversaw a fee-for-service hospital? In other words, where are the problem areas on which you would focus your attention, and who might provide ideas for “best practices” based on their performance?
  • What two actions would you recommend be taken at the mid-year point if you oversaw a capitated hospital? In this case, the revenue spreadsheet would be replaced with an overall budget of $50 million with which to operate (rather than being able to bill for each episode of patient care). Federal, state, county, and city hospitals normally operate under a capped budget. Additionally, many HMOs also operate under a fixed per member, per month (PMPM) capitated process.
