For example, you may choose to assess the period of young adulthood. Make sure you present the specific behaviors to support your clinical diagnosis, particularly the behaviors that correlate with the DSM 5 criteria. Use the following outline to organize your paper:

Section I: Initial Intake

I. Client Background

Present the client’s background and include age, gender, family history(mental heath, relationships, significant events).

II. Presenting Problem
Describe the problem or symptoms of the client that are negatively impacting the client’s life. Include physical and emotional symptoms. Note any thoughts or perceptions of the client that are related to the presenting problem.
III. Screening Tool
Identify a screening tool that you would employ in your screening process. Explain why this tool would be useful in your assessment.
IV. Diagnosis
Present your clinical diagnosis using the DSM 5(include the appropriate diagnostic code). Explain how the criteria meets the symptoms and behaviors. Also, describe any differential diagnoses and/or need for further information.
Section 2: Treatment Plan
Treatment Goals
Identify the long-term goals to address the symptoms and behaviors of the diagnosis. Then identify the short-term goals using SMART(Specific, Measurable, Achievable, Relevant, and Time Specific). These goals should directly address the problem symptoms or behaviors.
II. Social Work Therapy/Intervention
Present a social work intervention that is an evidence-based approach to treating this diagnosis. The intervention should address each of the short term goals/objectives. Explain how you would employ this intervention with this client, including the possible exchange in the treatment session(s) and possible client responses. Address any potential ethical issues.
