Re:Topic 1 DQ 1

Scientific management theory was developed by a theorist called Frederick Taylor, he developed this theory to improve the work efficiency. (Dininni, 2017) This theory asserts that every management body must identify the best ways to get the work done efficiently, this includes ways to train workers who are would competent to produce the best in what they are assigned to do. Setting up a reward system for the workers to improve their productivity is also one of the main goals for this theory (Dininni, 2017)

Hospitals could use this theory to design their management structure, staffing and units to care for a specific number of patients. A nurse manager would utilize this theory to evaluate nurses’ performances based on the set objectives, evaluate the number of patients seen, and documentation details. (Feigenbaum, 2017) This theory strongly believes that incentives and rewards are the driving forces behind good performance and increased productivity. When a worker is rewarded, he puts in his/her best.

Inefficient routines

One of the routines I noticed working in the nursing home is that most of the times, some of the prescribed medications are not reviewed for several weeks or even months, especially topical medications. The nurses would be signing in the MAR that the medication was given for a very long time even though the patient’s symptoms are no longer there, simply because the medication was not reviewed by the prescriber and discontinued. Another problem is the lack of communication among the nurses, some nurses forgets to document important information about patients care sometimes, for example patient’s follow-up appointment and lab results. The management decided to eliminate the unit managers due to financial problems, then shifting the workloads of the managers to floor nurses who have huge workloads, causing some nurses to quit, then creating staff shortage.

Participative decision

Involving everyone including the house keepers to spot and report change in condition on the residents. This was the idea of the nurses to involve other staff to report any change of condition using “stop and watch paper”. For example, if you notice a resident was not limping yesterday, but limping today, you must report that to the nurse for assessment. This effort has helped identify even small skin breaks for prompt intervention.

Re:Topic 1 DQ 1

One routine in health care that continues to be inefficient is the staffing ratios in Nursing. In Fredrick Taylor’s study of people, tasks, and work behavior, the goal was to improve the efficiency of the work process and labor relations (Dininni, 2017). However one must wonder if he had considered the Nursing profession. The Nurse to patient ratio continues to be inefficient, as evident by the ongoing nurse shortage (including CNA). Even though many institutions are offering heafty sign on bonuses and benifits packages, there still remains a shortage and high turnover rates because of the heavy workloads that don’t allow for safe, quality patient care.  Many staff voice concerns over not having the time devote to alot of the educational needs of patients because they just don’t have the time. An example of participative decision making in our clinic is self scheduling. When staff feel like they control of their schedules they tend to not call in as much because they feel as though they have some kind of control over when to schedule appointments, etc.

Scientific Management Theory’s

main objective is to improve performance in the work places. There are advantages and disadvantages to most things, which include the Scientific Management Theory. Advantages of Scientific Management include enhanced teamwork, cooperation between managers and workers, better planning and decision-making, improved democracy in workplace and improved physical working conditions for employees. Some disadvantages and ways this theory can be ineffective are managerial decisions may involve strictness and stress for managers due to need for complete control over the work place, influences feelings of pressure among workers and it may de-motivate workers (Advantage & Disadvantage of Scientific Management Theory | Study Psychology Online, 2013) An example of how it can de-motivate employees and be ineffective is the management tells the workers that they are now to perform a workflow a certain way. There’s no explanation, there’s not wiggle room, that’s just the way it is. The workers may feel frustrated! When a change is put into effect, an explanation of why the change is needed is helpful for the change to occur. If there is no chance for the workers that are actually going to perform the workflow to give input, they are de-motivated.

An example of participative decision making that has occurred in my workplace is allowing nursing staff to have input on new workflows that are developed. We have an opportunity to give ideas and insight to the IT department and the leadership. Although everyone’s input is taken into consideration, we aren’t allowed the final say which is what participative decision making is all about. The good think about this process is employees do feel valued and feel that their opinion matters. Another way that this occurs
is nursing staff from each team is allowed to be involved in the interview process for adding members to their pod.
