PURPOSE

This is an individual assignment completed over the course of 7 weeks. The assignment takes a systems approach and requires you to examine the functional pieces of a health delivery system in the United States. Being able to analyze services and systems in addition to organization and functions is an important competency. Developing a framework enables you to analyze the structure and organization of services and systems. The project enables you to apply concepts, theories and models using an applied and integrated systems approach. You select a large city or healthcare system and then study the various system features, inputs, outputs and resources. This project enables you to demonstrate skills, knowledge and competencies needed to be a successful healthcare manager.

In order to complete this project, you need a framework that provides a conceptual base to examine the structure and organization of the healthcare system. This assignment draws upon a systems framework as developed by Leiyu Shi and Douglas Singh (2015) in their book Delivering Health Care in America: A Systems Approach (6th Edition). In this assignment you examine the size and scope of the system, the basic components of health services delivery and major system characteristics. The assignment further examines trends, directions, concepts of health, new innovations, health status and health risks. You must develop a comprehensive analysis in a detailed report.

Your professional paper must reflect graduate level work, writing skills, research efforts, critical thinking and analysis. The report must be typed and double spaced. You must use headings and subheadings as you develop your systems framework. The paper must be clear and concise with no spelling or grammar errors. You will use the publication style of the American Psychological Association, Publication Manual of APA, 6th edition, 2009. Failure to submit an accurate, well organized, and comprehensive professional report will result in a reduced grade. All sections of the paper must be submitted for examination by the due dates so designated. Your completed paper should not exceed 50 pages and is submitted at the end of week #7.

The assignment has specific content areas and requires you to describe in detail each CHSSA section. You must address all 11 sections.

5.Describe system resources to include health workforce and health technology (Week #4).

6.Describe system outcomes including patient safety, quality of health, health policy, reducing medical errors, etc. (Week #4).

7.List and describe the impact of the Patient Protection & Affordable Care Act (PPACA).

8.Describe efforts related to public health and population based healthcare; community needs assessment; and other special initiatives.

9.Describe the development of Accountable Care Organizations (ACOs), Patient-Centered Medical Homes (PCMH), other relevant external forces affecting healthcare deliver.

10.List and describe forces of health reforms, market consolidations, mergers, acquisitions, bundled payments, funded research, value based healthcare, etc..

11.Describe and discuss issues of social justice, equitable distribution of health care, health care ethics, cultural beliefs and values, and Healthy People 2020 initiatives.

FACTS Ms. Hoffman was admitted to the hospital with an order for a renal arteriogram. After her admission to the hospital, Ms. Hoffman was presented with a consent form for the procedure. She was 84 at the time of admission and dealing with the onset of Alzheimer’s. Her husband Bob Hoffman was 86 at the time of her admission. He appeared to be competent but was dramatically overwhelmed by all the events transpiring around him and was very upset. As a result the hospital contacted the family of the two. They called their married daughter, Gisella, whose nickname is Goose. She arrived at the hospital with her husband Neil Down. Her married name is Down (Goose Down) as well. Their son, Upsan, (Upsan Down) was with them. He was 15 at the time. When the arrived at the hospital they were in a rush and slipped on some wax which was left by the maintenance people on the floor. Upsan went down and fractured his left femur. Upsan was rushed into surgery dues to the nature of the fracture, (compound) and the fact that the femoral artery was compromised. During surgery a decision was made that in order to save the leg that the femur would have to be reset and his leg would be approximately .5 inches shorter. The consent form listed five radiologists on the form but did not specify which radiologist would perform this procedure for her. The list of radiologists was composed of members of the Pinehurst Radiology Group. This group would determine which radiologist would cover the hospital each day. A consent form for Upsan was not used. Dr. Lina was assigned to perform Hoffman’s procedure. Following the renal arteriogram, the doctor determined that an angioplasty was necessary. There is significant evidence that the radiologist was in error and that the angioplasty was not necessary. In any event, the angioplasty was performed and because of some complications during the procedure, the patient had to be transferred to a different hospital. Her condition deteriorated within thirty days and eventually she lost significant motor functions. She is bed ridden. Dr. Lina has limited privileges at the hospital. Specifically, she is part of the Pinehurst Group, pays her own medical malpractice insurance and simply does the anesthesia/other services with the privileges of the hospital. After the surgery, while being transported to another recovery room, an attendant dropped her while transferring her to another gurney. She sustained a fractured hip.
