You are a consultant specializing in policy analysis. Based on the Examination and Treatment for Emergency Medical Conditions and Women in Labor legislation, as well as the situation that follows, you will complete a policy analysis with 3–5 options for your client, Congresswoman Moody, to consider.

Congresswoman Moody represents a state that borders Mexico. She is up for re election next year, and she will seek another term in office.

 

There are many undocumented workers that reside in her district. Congresswoman Moody is vocal about the need to provide health care to all that need it, but she also believes in fiscal restraint and does not support bail-outs for private facilities. She is well aware that her state’s Medicaid budget is almost exhausted for this year, and the state’s unemployment rate remains stubbornly high.

Apart from the voters, other affected constituents include three private regional medical centers trauma units that receive referrals from five or more small facilities that have emergency departments with lesser trauma status. Also, the largest health care corporation that owns two of the three medical centers in her catchment area, and which supported her in her last bid for election with campaign funds at the allowable limit, is threatening to refuse Medicare and Medicaid patients to ensure survival in the bad economy. An increase in unfunded mandates for urgent care may push this corporation into private payer only, and Congresswoman Moody is aware that there are enough affluent families in the area to support two facilities providing only designer medicine and concierge services.

What are the positive and negative aspects of managed care?

In the United States, managed care is becoming an increasingly popular method of administering healthcare. It influences the clinical behavior of providers, as it combines the payment and delivery of healthcare into a single system, the purpose of which is to control the cost, quality, and access of healthcare services for a single bracket of health plan enrollees (Scutchfield, Lee, & Patton, 1997).

Yet, managed care often evokes strong or negative reactions from healthcare providers because they are paid a fixed amount for treating their patients, regardless of the actual cost, which may influence their level of efficiency. This can challenge the relationships between doctors and patients (Claxton, Rae, Panchal, Damico, & Lundy, 2012; Sekhri, 2000).

Research managed care’s inception and study some examples. Be sure to investigate the perspectives about managed care from the vantage of both healthcare providers and patients. You can use the following keywords for your research—United States managed care, history of managed care, and managed care timeline.

Based on your research, answer the following questions in a 8- to 10-page Microsoft Word document:

  • What are the positive and negative aspects of managed care? Analyze the benefits and the risks for both providers and patients, and how providers should choose among managed care contracts. Conclude with your analysis and recommendations for managed care health plans. Your response should include answers to the following questions:
    • Summarize the history of when, how, and why managed care was developed.
    • Define and discuss each type of managed care organization (MCO)—health maintenance organization (HMO), preferred provider organization (PPO), and point of sale (POS).
    • Explain the positive and negative aspects, respectively, of managed care organization from the provider’s point of view—a physician and a healthcare facility—and from a patient’s point of view.
    • Explain the three types of incentives for providers for efficiency in the delivery of healthcare services. Explain who bears the financial risk—the provider, the patient, or the managed care organization.
    • Offer your recommendations, to accept or decline, for patients considering managed care health plans, with your rationale for each.

The initial step in developing your strategic management plan project is to select a health care organization. For more information on organization options, refer to the course project description. After you have determined an organization you want to use for the project, begin analyzing the following factors that affect your particular type of health care system:

  • Key socioeconomic factors: the demographics of the patient base, employee base, and physician base. These can include references to the aging population, increasingly ethnically diverse population, and staffing or physician surpluses and shortages.
  • Legislative factors: anticipated incremental legislative reforms versus large-scale reforms and legislative agendas aimed at reducing costs. Check both federal and state legislative actions.
  • Policy factors: regulations and policies external to the organization via government agencies and the Joint Council on the Accreditation of Healthcare Organizations.
  • Organizational factors: the overall organizational structure, the emphasis on recruiting and retaining talent, and the emphasis on staff development and competency building.
  • Service delivery factors: causes of change in care delivery due to the evolving practice of medicine, advances in pharmacology and technology, and the emphasis on cost-effective care.
  • Technological factors: medical equipment development, research advances, and information technology systems implementation.
